You know that a pony is a small (little) horse, but the other end of the joke is referring to Spitfire's voice. In this episode of the show her voice was raspy and had a more gruff sound to it, and when that happens it's sometimes referred to as "sounding horse". People have a tendency to sound horse when they're sick because their throats hurt, which is why RD asked her if she was feeling ok.
